Acetabular Revision in Massive Bone Defect using Antiprotrusio Cage

Abstract
|
|
|
Purpose: To evaluate the clinical and radiographical results of an acetabular revision for massive acetabular bone deficiency using an antiprotrusio cage (APC) and cryopreserved morcellized allograft.
Materials and Methods: Twelve acetabular revisions, using an APC and morcellized allograft, were performed for massive acetabular bone deficiency between October, 1998 and April, 2001, and were followed up for at lease 24 months. The clinical and functional grades were evaluated by Harris hip scores. The radiological evaluations focused on the location of the center of rotation.
Results: The mean postoperative Harris hip score was 46.3, which improved to 86.2 following the revision, with the exception of one Girdlestone hip resection case. Radiographically, the center of rotation was repositioned laterally and downward by 4.8 and 9.2mm, respectively. At the last follow up, the center of rotation had not migrated from the new position in any of the revisions. There were 3 hip dislocations and 3 infections as postoperative complications.
Conclusion: With massive acetabular bone deficiencies, revision using an APC with a morcellized allograft is a good treatment method. However, there is a need to pay attention in order to prevent postoperative infection and dislocation.
